Open your tube and choose two colors from it:

Foreground : dark

Background : light.

Change the background to gradient,

with these settings:

  
Open a new transparent image of 500 x 400 pixels.

Fill with the gradient.

Duplicate this layer.

Selections -- Select All

Effects -- Filters Effects --

Border Mania -- Frame #2

with the top two sliderbars on 3-1

Deselect.

Image -- resize to 80%

"reduce all layers" NOT CHECKED

Effects -- 3D Effects  -- Drop Shadow,

with these settings:

Add a new raster layer

and fill with your gradient,

Layers -- Load/Save Mask --

Load Mask from Disk,

and find the Nadia_art_mask1

Apply with these settings: 

   
Delete the mask layer in the layers palette

and reply "yes" to the question.

Layers -- Merge -- Merge Group

Effects -- 3D Drop Shadow,

with these settings:

   
Effects -- Image Effects --

Seamless Tiling,

with these settings:

  
Add a new raster layer

Selections -- Load from Disk --

Find the joyce45 selection and apply.

Fill the selection with the dark color twice,

by clicking twice in it.

Effects -- Texture Effects -- Blinds,

with these settings:

Effects -- Edge Effects  -- Enhance

Reduce the opacity of this layer to 50%

in your layers palette.
 

Add a new raster later

and fill with your gradient.

Layers -- Load/Save mask from Disk --

Load mask -- find the GDMask2 and apply.

Delete the mask layer

and repeat "yes"to the question.

Layers -- Merge -- Merge Group

Adjust -- Sharpness -- Sharpen.

Repeat this step once.

  Duplicate this layer

Image -- Flip

Image -- Mirror

Reduce the opacity of this layer to 50%

Add a new raster layer

Selections -- Load from Disk,

and find the joyce46 selection.

Apply.

Fill with your dark color.

Deselect

Effects -- Edge Effects -- Enhance

Reduce the opacity of this layer to 25%

in your layers palette.

 
 Add a new raster layer.

Selections -- Load from Disk --

find the joyce41 selection and apply.

Fill with your dark color.

Reduce the opacity of this layer to 50%

Duplicate this layer.

Image -- Flip. 
  
Layers -- New raster layer

Fill with your gradient.

Selections -- Select All.

Effects -- Filter Effects --

Border Mania -- Frame #2,

with the top two slidebars on 3-1.

Selections -- Select None.

Image -- Resize to 80%

"reduce all layers" NOT CHECKED.
Repeat the resize effect 2x with the same settings.

Effects -- Texture Effects -- Mosaic Glass,

with these settings:

 
  
Effects -- Filter Effects -- Fm Tile Tools --

Blend Emboss - standard

Effects -- 3D Effects --

Drop Shadow - same settings as before.

(Blur should be on 1)
 

Add a new raster layer  
Selections -- Load from Disk,

find the joyce48 selection and apply.

Fill with your light color.
 

Effects -- 3D Effects --

Drop Shadow with the same settings as before.

Selections -- Select None.
 

Add a new raster layer.
 Activate your Brush tool,

and find the  "aravensbrush!poem!LOVER"
  

Click just below the top line

with your dark color

(you can even use your light color

if you prefer it)

See my example above
 

Add a new raster layer.

Set your background on your dark color

and lock your foreground.
 

Activate your Text tool

and choose the "Splendid ES"- font, size 125.

Type the first letter of your name in uppercase

and drag it into place.

Reduce the opacity of this layer to 80%

Add a new raster layer,

type the rest of your name, with font size 60.

Reduce the opacity of this layer to 80%.
  

Activate your tube.

Resize to 400 pixels in height.

Edit - Copy

Edit - Paste as new layer onto your canvas.

Drag it to the left and position it.

Effects -- 3D Effects -- Drop Shadow,

with these settings:
 

Layers -- Merge -- Merge All.

Effects -- Filter Effects --

Border Mania -- Frame #2,

with the top two slidebars on 3-1
 
And you're finished ;-)
